§ Medical Power of Attorney - This document appoints someone of your choice to make medical decisions for you in the event you're unable to make them for yourself.
§ Directive to Physician (Living Will) - This document states clearly your choice of how doctors, et al are to proceed in the event of catastrophic illness or injury where death is imminent. Sometimes crudely referred to as the pull-the-plug-or-not choice.
